What is Fissure?

An anal fissure is a small cut or tear in the anus lining. During and after bowel movements, the crack in the skin causes severe pain and some bright red bleeding. The fissure can sometimes be deep enough to expose the muscle tissue beneath.

Fissures can happen to all age groups and can be managed with a healthy diet & simple treatments. Few people might need to undergo occasional surgery for the best treatment of anal fissures.

Symptoms

Acute pain in anal region/area
Itching and burning sensation in the anal area
Formation of small skin tags close to the fissure
Severe pain during bowl movements
Blood in stools
Small tear / cut around the anus

Causes

Less blood flow to rectal area
Tight anal sphincter muscles
Anal intercourse
Hard stools and constipation
Tuberculosis
Anal cancer

Who is at risk?

  • Less blood flow to rectal area
  • During infancy, anal fissures are common. Because of decreased blood flow in the anorectal area, older adults are also prone to anal fissures. Women are at risk for anal fissures during and after childbirth due to straining during delivery.
  • IBD patients are also more likely to develop anal fissures. Because of the inflammation in the intestinal lining, the tissue around the anus is more prone to tearing.
  • People who have constipation regularly are more likely to develop anal fissures.

Treatment

Anal fissures usually heal up if proper diet and exercise are followed regularly; which helps soften stool. One should sit in warm water for 15 to 20 minutes multiples time a day to relax anal muscles. If done after bowl movements, it helps in healing. Persistent symptoms again need extra attention and your specialist might suggest and recommend non-surgical as well as surgical treatment.

Non Surgical Treatmen

  • Application of anaesthetic creams, such as Xylocaine helps in relieving pain to a great extent
  • Botox injections are used to relax the spasm in the anal area.
  • Oral medications are also suggested to help relax anal sphincter muscles.

Surgical Treatment

Severe symptoms are often treated surgically. The ideal line of treatment is Fissurectomy; wherein the edge of the fissure is surgically removed along with skin tags around the area. Hospitalization is usually for 1 day and recovery is faster.
